TxDOT says an applicant seeking to construct or revise a driveway should contact the local district office and submit Form 1058 with the proposed work, applicant details and driveway location.

Give the closer a packet that sorts source clues from conclusions.

The best driveway and utility handoff is not an access opinion or service decision. It tells the team what source exists, what process or file condition raised the issue and who needs the next answer.

  1. Property address, county, parcel, frontage road or highway clue, subdivision or development name, transaction type and target timing
  2. Driveway lane: existing driveway, proposed driveway, modified driveway, TxDOT-maintained roadway clue, Form 1058 status, approved permit copy or unknown status
  3. Utility lane: water provider, electric provider, utility will-serve letter, RULIS or utility-permit clue, easement or right-of-way source and who owns the next answer
  4. County-development lane: plat package, OSSF review, ETJ platting verification, drainage or TIA source, tax certificate, deed and surveyor PDF if those are what raised the question
  5. Title packet lane: commitment or GF number, Schedule B or C item number, survey reference, recorded instrument, lender condition, attorney note and one process question

Keep access, utility, underwriting and legal conclusions out of the first note.

  • Do not use the public form to decide access rights, driveway approval, utility availability, utility capacity, easement scope, permit sufficiency, plat approval, title status or legal effect.
  • Do not assume a will-serve letter, RULIS entry, driveway permit, plat package, survey, deed, tax certificate, commitment item or lender note satisfies a county, utility, title, lender or underwriting requirement.
  • Do not paste wire instructions, account numbers, taxpayer credentials, Social Security numbers, identity-document numbers, confidential contracts, complete commitments, surveys, plats or client documents.
  • Do not offer or request a rebate, discount, commission, gift or any other benefit for sending the question or future closing.
